"The Advanced Heart Failure Program at UMass Memorial Medical Center constantly monitors care and addresses the needs of our heart failure patients. A multidisciplinary task force meets regularly to review our patient outcomes and quality data, identifies areas for improvement and implements changes. The length of stay, mortality rates and readmission rates are monitored and measured against the best rates in the country, to ensure we are doing everything possible for our patients.
"One of our improvement initiatives is to have all our heart failure patients cared for on a single patient care unit. Here they benefit from care by specifically trained staff. Using a team approach to deliver care, we ensure that patients receive appropriate care and testing, as well as education about their illness and instructions when they leave the hospital."
Theo Meyer, MD, DPhil, is the director of the UMass Memorial Advanced Heart Failure Program and Cardiac Catheterization Labs. He is also a professor of medicine at the University of Massachusetts Medical School.
